PREFACE Dear Colleagues, It is an honour for us to welcome you to Bodrum, Turkey for the 22nd INTERNATIO- NAL SCIENTIFIC CONGRESS of TURKISH ASSOCIATION of ORAL and MAXILLO- FACIAL SURGERY. We would like to thank to the lecturers of our congress who accepted our invitation from all around the world and also to our colleagues who supported us with 31 oral presentations and 222 poster presentations. As the only association representing Turkey in central and regional scientific area of the world in the field of Oral and Maxillofacial Surgery, we are glad to see this great participation to our meeting. INVITED SPEAKERS Dr. PUSHKAR MEHRA, USA CURRICULUM VITAE Dr. Mehra is an Associate Professor in the Department of Oral and Maxillofacial Surgery at the Boston University Henry M. Goldman School of Dental Medicine. He graduated valedictorian, both from King George s Medical College, Lucknow, India, and GSDM. He completed his Oral and Maxillofacial Surgery Residency from the combined training program between Boston University and Tufts University. Following graduation, he did a fellowship in Maxillofacial Reconstructive Surgery under the tutelage of Dr. Larry Wolford at the Baylor University Medical Center in Dallas, Texas. His clinical interests include surgical treatment of obstructive sleep apnea, maxillofacial pathology, nerve repair, and complex functional and esthetic facial reconstruction including Orthognathic, TMJ and Cosmetic Surgery. Dr. Mehra has over 60 publications in medical and dental journals and textbooks and is frequently invited to lecture at various national and international meetings. He is a Fellow of the American Association of Oral and Maxillofacial Surgeons, and a Diplomate of the American Board of Oral and Maxillofacial Surgery. He was elected in 2009 as a Massachusetts delegate to the American Association of Oral and Maxillofacial Surgeons and is currently serving his second consecutive term in that position. Dr. Mehracurrently serves as a member of the Examination Committee of the American Board of Oral and Maxillofacial Surgery and was recently appointed as Section Editor, Section 1 by the American Board of Oral and Maxillofacial Surgery in 2013 for a two-year term. He is also a consultant to the Commission on Dental Accreditation and was previously.held the position of Section Editor of the Cosmetic Surgery section of the OMSSAT/OMSITE examination from 2004-2008. 9

INVITED SPEAKERS S Advanced techniques in orthognatic surgery: segmental surgery and counter-clockwise rotation Modern concepts and surgical techniques, improved armamentarium and lessons learnt from our experiences and mistakes have allowed us to resolve some of the past clinical controversies in orthognathic surgery. We can now predictably manipulate the maxillofacial skeleton in three dimensions of space and achieve movements that that were previously thought as not possible or predictable. In this presentation, we will review contemporary corrective jaw surgical techniques related to maxillary and mandibular surgery. The two specific concepts, which will be presented, are: 1) Segmental maxillary surgery, and, 2) Counterclockwise rotation of the maxillomandibular complex. Specific surgical modifications and hard and soft tissue manipulation techniques aimed at allowing for more predictable surgical results, especially in cases with complex movements will be presented. Corticotomy-assisted orthodontics treatment Adult orthodontics is becoming very popular and an increasing number of adults now undergo orthodontic treatment. Duration and cost of the treatment still remain the biggest challenges for the orthodontic field in the current era relative to treatment of adults. Typical orthodontic treatment ranges from 1-2 years, which requires continuous compliance and expense from the patient s perspective for a considerable period of time. Although, a relatively new technique, selective alveolar decortication (SAD) has the potential to provide a solid platform for improvement in the efficiency and effectiveness of current-day orthodontic therapy. Besides increasing the scope of orthodontic treatment, it hastens the results of conventional non-surgical orthodontic therapy and has the potential to correct certain specific malocclusions with minimally invasive, office-based surgery. This presentation details experiences of a single clinician with over 100 patients with this technique and aims to familiarize the audience with the biophysiology, surgical technique, advantages, disadvantages, indications, contraindications, and limitations of SAD. 10

INVITED SPEAKERS Dr. Edward Lahey, USA CURRICULUM VITAE Dr. Edward Lahey hails from Connecticut, where he received his undergraduate degree from Fairfield University. He received his DMD summa cum laude from Tufts University School of Dental Medicine and then completed the year-long Clinical Research Training Program at the National Institutes of Health in Bethesda, Maryland. Dr. Lahey obtained his MD from Harvard Medical School while completing his residency in oral and maxillofacial surgery at Massachusetts General Hospital. Following his specialty training, he joined the faculty of HSDM and the Harvard oral and maxillofacial surgery faculty practice. Dr. Lahey is a diplomate of the American Board of Oral and Maxillofacial Surgery. His research-oriented practice focuses on the surgical treatment of adult and pediatric jaw abnormalities, wisdom teeth, dental implants, maxillofacial trauma, and pathology. 11

INVITED SPEAKERS Obstructive Sleep Apnea: An update on its physiology, evaluation and surgical treatment Obstructive sleep apnea (OSA) is the most common sleep-related breathing disorder, with a prevalence of at least 15% in men and 5% in women. The recurrent loss of airway support and subsequent decreases in oxygen saturation and fragmentation of sleep that occurs in this chronic condition have been implicated in the development of hypertension, obesity, increased road traffic accidents and early death. While continuous positive airway pressure (CPAP) is the gold standard treatment for OSA, upwards of 50% of patients will reject this treatment within the first few months of institution, leaving a large cohort of patients without treatment. Various surgical interventions have been attempted to meet the needs of these patients. To date, the most successful of these is maxillomandibular advancement (MMA) which is based on the techniques developed to treat dentofacial deformities. While it is clear that MMA is very successful in treating OSA, there are many questions about this intervention yet to be answered. This presentation will provide a review of basic sleep physiology, the physical examination and studies used in the work up of the OSA patient, and the current state of surgical planning and execution. There will also be a review of the Massachusetts General Hospital experience in treating OSA. Surgical management of lingual nerve injuries Iatrogenic trauma of the lingual nerve during extraction of impacted mandibular third molars is the complication most feared by surgeons and patients alike. Studies indicate that at least 75% of oral and maxillofacial surgeons will cause a lingual nerve injury during third molar extraction at some point in their career. While lingual nerve injury may never be completely avoidable, incorrect and poor management of an injury is. Application of peripheral nerve injury principles to lingual nerve trauma may prevent an unfortunate but potentially modifiable outcome from becoming a permanent disability. This presentation will provide a review of the risk factors for lingual nerve injury, the appropriate evaluation of a patient with a lingual nerve injury and the principles of management. A focus on surgical technique used to treat a lingual nerve injury will be given along with a review of the Massachusetts General Hospital experience in treating lingual nerve injuries. 12

INVITED SPEAKERS DR. GERHARD UNDT, AUSTRIA CURRICULUM VITAE Professor Undt, based in Vienna, is a specialist for oral and maxillofacial surgery as well as for dental and oral medicine. Since 1992 he has specialised in the field of dysfunctions of the temporomandibular joint, or the TMJ. During his time at the Viennese University Dental Clinic he developed a particular interest in splint therapy and electronic recording of TMJ function. During his surgical training he covered the entire range of open and endoscopic maxillofacial surgery. He specialised very early on in the surgical treatment of functional TMJ diseases and of fractures to the condylar process of the mandible. Professor Undt has been performing arthroscopic TMJ surgery since 1994, gaining rich and varied experience in the wide range of arthroscopic microsurgical techniques, with instruction from international specialists. Nowadays Professor Undt himself is among the world s leading specialists in the field of arthroscopic and reconstructive TMJ surgery. Not only does he perform surgery in Vienna, but also in university clinics all over the world as a guest professor. His numerous live-operations that are transmitted to international congresses and symposia have helped to familiarise a large group of specialists with minimally invasive surgical techniques for TMJ surgery. Since 2003 Professor Undt has held a TMJ surgery course in Vienna - this is now the most popular course in this specialist field worldwide, and has to date attracted over 400 participants from 50 nations from all corners of the globe. Since 2003 Professor Undt has also focused on the endoscopic treatment of salivary gland diseases. Diagnostic and interventional sialendoscopy allows salivary gland stones to be removed with minimally invasive microsurgery, rather than having to remove all or part of the salivary gland. More and more patients are coming to Vienna to undergo this specialist surgery. 13

INVITED SPEAKERS S Minimally invasive TMJ surgery Arthroscopic surgical techniques have changed the therapeutic approach of functional disorders of the temporomandibular joint significantly. A wide variety of microsurgical techniques that allow precision surgery to be performed on soft and hard tissues in the temporomandibular joint, have been developed over the past 20 years. Mechanical hand-held instruments, diathermy probes, laser probes and electrically driven shavers are used to separate adhesions within the joint space and to reconstruct damaged cartilage and bone. According to large-scale international studies, the success rate of this minor surgical procedure is reported to be over 90% in terms of reduction of pain and restoration of function. However, in contrary to open TMJ surgery, surgical arthroscopic techniques - like other minimally invasive surgical procedures - require intensive training in order to avoid possible complications. Reconstructive TMJ surgery For the treatment of ankyloses of the temporomandibular joint special surgical techniques that prevent further apposition of heterotopic bone are required. Alloplastic devices are the current gold standard in TMJ reconstruction, but biological methods are still widely used. We present our technique of temporomandibular joint reconstruction by using extra-corporally augmented autologous chondrocytes for resurfacing of bone of the condylar head, the glenoid fossa and the articular eminence. This technique of matrix associated chondrocyte transplantation (MACT) has been used in knee surgery since the early 1990s and has now been adapted for the special use in the TMJ. We also have modified the pre-auricular approach to the TMJ in order to avoid visible scars, impairment of the blood supply of the auricle and impairment of the regional innervation. 14

Continuing research, new materials, the rising expectations of the patient are the main forces that drive us to seek solutions able to formulate more predictable and less invasive aesthetic and functional rehabilitation plans. The BoxTechnique is the first prosthetically and aesthetically guided boneregenerative technique that allows to regenerate bone in the three space dimensions by using only absorbable materials which are processed naturally by the body. INVITED SPEAKERS S Role of Oral and Maxillofacial Surgeons in Cleft Lip and Palate Treatment Cleft lip and palate treatment is a long-term process in which numerous departments/ wards are involved. Whereas the approach of specialties such as prenatal diagnosis, genetic consultation, social counseling and speech therapy are well defined; the surgical and some of orthodontic treatments differ in technique, sequence and timing. Consequently, the operations should be performed by surgeons who have profound knowledge of dentistry, experience regarding the cleft patients beyond the surgical process and they should be able to recognize potential problems during growth. Principles of cleft lip and palate repair Primary repair of cleft lip and palate has to keep in mind that there are inborn and induced forces, which influences further growth.
