Foreword Önsöz ÖNSÖZ Halk sa lı ı istatistiklerinin e itici, koruyucu ve tedavi edici çalı malara dayandı ı bir gerçektir. Bu hizmetlerin yerine getirilmesi, yurdun sa lık durumunun ve seviyesinin bilinmesiyle yakından ilgilidir. Sa lık sorunlarına pratik çözüm yollarının bulunması, en azından toplumda sa lık olaylarını temsil edebilecek seviyede tam ve güvenilir istatistiki verilerin derlenmesi ve bunların bilimsel anlamda de erlendirilmesi ko uluna ba lıdır. Sa lık Ara tırması sa lık alanında idari kayıtlar yoluyla derlenemeyen bir çok göstergenin üretilmesi ve bu konuda karar alıcılara ve ara tırmacılara veri kayna ı olu turmak amacıyla uygulanmı tır. Be yıldan daha uzun bir süredir Avrupa Birli i statistik Ofisi tarafından Avrupa Birli i ne üye ve aday ülkelerde Sa lık Ara tırması nı uygulayabilmek için model hazırlama çalı maları sürdürülmü ve bu konuda hazırlanan düzenleme, Avrupa Parlamentosu ve Konsey de Avrupa Birli i düzeyinde sa lık ara tırması verilerini de içerecek ekilde Halk Sa lı ı statistikleri alanında kar ıla tırılabilir veri elde edilmesini tüm üye ülkeler için zorunlu hale getirmi tir. Ara tırmaya, Avrupa Birli i statistik Ofisi tarafından üye ve aday ülkelere önerilen modüllerin yanı sıra ulusal ihtiyaçları da kar ılamak amacıyla bölümler eklenmi tir. Ara tırma da, 0-6 ve 7-14 ya gruplarındaki çocukların anket tarihinden önceki 6 ay süresince geçirdikleri hastalık ve kazalara ait bilgiler ile bu rahatsızlık için ne tür hizmet aldıklarına dair verilere ula ılmı tır. 15 ve daha yukarı ya taki bireyler için genel sa lık durumu, kronik hastalıklar, günlük aktivitelerini yürütmede fonksiyonel yeterlilik, ki isel bakım, sa lık hizmetleri kullanımı, ilaç kullanımı, yapılan a ılar ve ölçümler ile boy ve kilo de erleri gibi bilgiler derlenmi tir. Ara tırmanın sonuçları Türkiye, kent ve kır ayrımında kullanıcıya sunulmaktadır. Yayının karar alıcılara, ara tırmacılara ve tüm kullanıcılara yararlı olaca ı inancı ile bilgileri veren hanehalklarına, ara tırmanın özellikle anket formunun geli tirilmesi sırasında de erli katkılarını ve desteklerini esirgemeyen üniversitelerin ö retim üyeleri ile ilgili kurum ve kurulu lara, özverili çalı malarından dolayı Türkiye statistik Kurumu çalı anlarına te ekkürlerimi sunarım. FOREWORD It is real that public health statistics based on instructive, preventive and curative studies. The fulfillment of these services is intimately related with health status of country and level of supplying of health care services. Finding of the practical solutions is connected to gather accurate and confidential statistical data that representing health situations in community level and also evaluation this data as scientific. Health survey was realized with the aim of providing indicators in health area not obtaining by administrative registration system and also constitute data source for the decision-makers and researchers. More than five years European Community Statistical Office has been preparing a model to apply Health Interview Survey in all member states and candidate countries and regulation that accepted in the European Parliament and the Council makes it mandatory for all member states and candidate countries to provide EU comparable Public Health Statistics. In the survey, all questions were applied covered in the modules that recommended by European Community Statistical Office and also for national requirements some questions were added to the survey. With the survey, the data was obtained with the diseases and accidents in the past 6 months before the survey date for the children in 0-6 and 7-14 age groups and kind of health care services that taken for these discomforts. The information was derived related with general heath status, chronic diseases, functional abilities in carrying out daily activities, personal care, using of health care services, using of medicines, vaccinations and height and weight values for the individuals in 15 years and old age group. Results of the survey are presented to the users in Turkey total, urban and rural discrimination. With the idea of that this publication will be beneficial for decision-makers, researchers and all other users, I would like to express my special thanks to households providing information, members of the universities, all related institutions who gave valuable contribution and support given in the improvement of the questionnaire and also Turkish Statistical Institute s staff for their selfsacrificing for this survey. Giri Sa lık Ara tırması ile bebek, çocuk ve yeti kinlerin sa lık durumunun yanı sıra 15 ve daha yukarı ya taki bireylerin sa lık hizmetinden faydalanma durumları ve memnuniyet düzeyleri, günlük aktivitelerini gerçekle tirirken ya adıkları zorlukların derecesi ile sigara ve alkol kullanma alı kanlıkları ba ta olmak üzere sa lık alanındaki birçok gösterge elde edilmektedir. Ara tırma nın öncesinde Avrupa Birli i tarafından sadece 15 ya ve daha yukarısı için önerilen modüllerin Türkçe ye çevirisi yapılmı tır. 2. Amaç 2010 Sa lık Ara tırması ile bireylerin genel sa lık profilinin ortaya çıkarılması ve ülkelerin geli mi lik düzeylerini gösteren kalkınma göstergeleri içerisinde önemli bir paya sahip olan sa lık göstergesine yönelik bilgilerin elde edilmesi ile mevcut yapıdaki bilgi açı ının kapatılması amaçlanmı tır. Ara tırma, hem uluslararası kar ıla tırmalara imkan veren hem de ulusal ihtiyaçlara ı ık tutan bir ara tırma olma özelli ini ta ımaktadır. 3. Kapsam 3.1 Co rafi Kapsam Türkiye Cumhuriyeti sınırları dahilinde bulunan tüm yerle im yerleri örnek seçimi için kapsama dahil edilmi tir. 3.2 Kapsanan Kitle Türkiye Cumhuriyeti sınırları içinde bulunan hanelerde ya ayan ve son bir yıl içinde sa lık hizmeti almı tüm fertler kapsanmı tır. Ara tırmada toplam nüfusun yakla ık %2 sini olu turan kurumsal nüfus kapsam dı ı bırakılmı tır (Kurumsal nüfus; okul, yurt, otel, çocuk yuvası, huzurevi, hastane ve hapishanede bulunanlar ile kı la ve ordu evlerinde ikamet eden nüfusu içermektedir). 1. Introduction Health Survey derives many indicators on health including health conditions of infants, children and adults and also the utilization of health services, satisfaction levels from these services, difficulties faced during daily activities and cigarette and alcohol using habits for individuals 15 years old and over. Before the survey implementation, the modules recommended by Eurostat for the 15 years and older age group translated to the Turkish. 2. Purpose The aim of the 2010 Health Survey is to introduce the health profile of individuals and to get information about health indicators which constitutes a big part of the development indicators that shows the degree of development of the countries and also to close the lack of information in current system. This survey has the specialty that held out to reflect the country in general and being comparable internationally. 3. Coverage 3.1 Geographical Coverage All settlements in the territory of Republic of Turkey were covered in sample selection. 3.2 Population Covered All the individuals living in Turkey and who received health service in the previous year were covered. In the survey, institutional population which consists of 2% of the total population were not included in the coverage (institutional population includes the population living in; schools, residents of dormitories, hotels, kinder-gardens, rest homes for elderly persons, special hospitals, military barracks and recreation quarters for officers). Tasarım gere i toplam örneklem hacmi 7 886 hane olarak belirlenmi tir. 4.1 Estimation Level and Sample Size The survey is designed in order to produce estimators for total of Turkey, urban and rural. Thus, the total sample size necessary was found to be 7 886 households. Örnekleme Yöntemi Tabakalı iki a amalı küme örneklemesi yöntemi kullanılmı tır. Dı sal tabakalama kriteri olarak kırkent ayrımı kullanılmı tır. Birinci a ama örnekleme birimi ortalama 100 hanehalkından olu mu bloklar, ikinci a ama örnekleme birimi ise her bloktan sistematik olarak seçilen hanehalklarıdır. Adres Çerçevesi Ara tırmanın örneklem çalı masında kullanılan çerçeve 2007 yılında tamamlanan Adrese Dayalı Nüfus Kayıt Sistemi (ADNKS) ne altlık olu turan Ulusal Adres Veri Tabanı dır (UAVT). Adres çerçevesi, UAVT den 2010 yılı Mart ayında alınarak olu turulmu tur. Nüfusu 133 ün altında olan yerle im yerleri, yeterli örnek hane sayısına ula ılamayaca ı dü ünüldü ünden kapsam dı ı bırakılmı tır. Tabakalama Kriteri Nüfusu 20 000 ve daha az olan yerle im yerleri Kır, 20 001 ve daha fazla olanlar Kent olarak tanımlanmı tır. Sampling Method Strata and two phase cluster samling method was used. For external stratification rural urban difference was used. First phase sampling unit were blocks consisting of an average of 100 households and second phase sampling unit were households selected systematically from each block. Address Frame The frame used in the sampling of the survey was the ''National Address Database'' (NADB) which constitutes a base for ''Address Based Registry System'' (ABRS) which was completed in 2007. (Adress frame was composed from NADB in March, 2010). Settlements with population less than 133 were not included in the frame because it was considered that adequate number of sample households wouldn't be reached. Stratification Criterion Settlements with population 20 000 and below defined as rural, settlements with population 20 001 and over defined as urban. Sampling Distribution In urban settlements; 5696 households were selected from 356 blocks which were previouslu selected (15 households in each block). In 4682 of these households the questionnaire was completed. In rural settlements; 2190 households were selected from 219 blocks which were previously selected (10 households from each block). Firstly, base weights which are inversely proportional to the overall selection probabilities were calculated for each sample respondent. In this stage, it was calculated probabilities of selection of clusters, selection of households and then base weights were calculated by using these probabilities based on household and individual. In the second stage of weighting, base weights were adjusted to compensate the losses in the sample outcome due to non response. In the final stage the weights are calibrated to the projected population distributions, using integrated calibration ratio method. The calibration is iterated and trimmed until any relative changes in the weights are within a pre-specified range. The final weights are then inflated by an overall inflation factor. 5. When the percentages analysed by urban-rural level; it is seen that Social Insurance Institution (SII) is taken the first place in urban areas with 49,9% and in rural areas green card has the highest value with 30,0%. 1
