1. INTENDED USE: Quantitative LC-MS/MS analysis kit for Epinephrine, Norepinephrine, Dopamine, Serotonin, Gamma-aminobutyric acid, Glutamic Acid, Histamine, 5-Hydroxy-L- Tryptophan in urine. 2. SUMMARY AND EXPLANATION: Zivak Neurotransmitters LC-MS/MS Analysis Kit was developed for accurate analysis of Epinephrine, Norepinephrine, Dopamine, Serotonin, Gammaaminobutyric acid, Glutamic Acid, Histamine, 5-Hydroxy-L-Tryptophan in urine samples. 3. TEST PRINCIPLE: Neurotransmitters are extracted from human urine samples by using an acidic extraction and deproteinization steps. After the extraction, these analytes derivatized via reagents. Then derivatizated Neurotransmitters analyzed by LCcoupled tandem mass spectrometry. 4. WARNING AND PRECAUTIONS: For in-vitro diagnostic use only. Read the instructions carefully, before you start. In case of damage of the kit package, please contact Zivak or your supplier. Do not use expired kits and components. Please check batch no and expiry date before you start. Although the concentration of all toxic components is low, for safety reasons the sample preparation station should be placed in an exhaust hood. Protective gloves and goggles should be worn. Please take any necessary precautions to prevent infection with blood borne pathogens while working with biological fluids. Appropriate bio-safety precautions and disposal of bio-hazardous wastes should be followed. Please check the labels on reagent bottles. Reagents of this kit contain hazardous material may cause eye and skin irritations. 3
5. STORAGE AND STABILITY: Reagent 1,2,3,4 and calibrators, controls should be stored at between 2-8 C. Other all components of the kit should be stored at room temp. Urine calibrators and control must be stored at -20 C after reconstitution. All components are guaranteed until expiry date when stored at recommended temperatures and used as described in this instructions. 6. PROCEDURE NOTES: Any inappopriate handling of samples or modification of the test procedure may influence the results. The indicated pipetting volumes, incubation times, temperatures and pretreatment steps have to be performed strictly according to the instructions. Use calibrated pipettes and devices only. Once the test is started, all steps should be completed without interruption. Make sure that required reagents, materials and devices are prepared ready at the appropriate time. Leave aside all reagents and specimens to reach room 5
temperature (18-25 C) and gently swirl each vial of liquid reagent and sample before use. Mix reagents without foaming. Avoid contamination of reagents, pipettes and wells/tubes. Use new disposable plastic pipette tips for each reagent, standard or specimen. Do not interchange the vial caps. Always keep vials closed when not used. Do not re-use wells/tubes or reagents. Incubation time affects results. All tubes or wells should be handled in the same order and time sequences. 9. LIMITATIONS OF THE PROCEDURE: Specimen collection and storage have a significant effect on the test results. Urine samples must be kept frozen or acidified with appropriate acid solution before the analysis. (6N HCl, %50 Acetic acid, Na2CO3 (crystals), Boric acid (crystals)). If the urine sample is not clear, it has to be centrifuged before sample preparation. 10. PRE-TEST SET-UP INSTRUCTIONS: Preparation of Zivak Calibrator Level-I: Add 8 ml 0,1 M HCl to urine calibrator and dissolve. After preparation, calibrator must be aliquoted into 120 µl volumes and stored at -20 C. Preparation of Zivak Calibrator Level-II: Add 8 ml 0,1 M HCl to Urine Control Level I and dissolve. After preparation, Control Level I must be aliquoted into 120 µl volumes and stored at -20 C. Preparation of Zivak Control Level I: Add 8 ml 0,1 M HCl to Urine Control Level II and dissolve. After preparation, Control Level I must be aliquoted into 120 µl volumes and stored at -20 C. 6
11. TEST PROCEDURE: Sample Pre-treatment (Automated) Take 100 L of patient sample (or calibrators and controls) into the Zivak Multitasker Sample Preparation vials Make sure that reagents and wash solution are connected to the reagent pump and purged properly. Place the vials on the MULTITASKER system tray. Note: The prepared sample is stable at 2-8 C for 24 hours. Sample Pre-treatment (Manual) Take 100 L of patient sample (or calibrators and controls) into the sample preparation tube. Add 100 L of Reagent 1 and vortex for 10 seconds. Add 200 L of Reagent 2 and vortex for 10 seconds. Add 200 L of Reagent 3 and vortex for 10 seconds. Wait for 2 minutes and vortex again for 10 seconds. Wait for 1 minute. Add 200 L of Reagent 4 and vortex for 10 seconds. After mixing wait for a minute. Centrifuge at 12.000 x rpm for 3 minutes. Take 200 L the upper phase into the autosampler vial. Inject 3 L to the LC-MS/MS system. Note: The prepared sample is stable at 2-8 C for 24 hours. 7
12. QUALITY CONTROL: The test results are only valid if the test is performed by following the instructions. Moreover the user must strictly adhere to the rules of GLP (Good Laboratory Practice) or other applicable standards/laws. All standards and kit controls have to be found within the acceptable ranges as stated on the QC Certificate. If the criterias are not met, the run is not valid and should be repeated. Each laboratory should use known samples as further controls. In case of any deviation the following technical issues should be proven: Expiry dates of (prepared) reagents, storage conditions, pipettes, devices, incubation conditions and washing methods. It is recommended to participate at appropriate quality assessment trials. 13. CALCULATION OF RESULTS: The obtained area of the standard is plotted against their concentration. The standard curve is calculated by a linear regression or a weighted linear regression function. Using computer programs, the curve is best described by a 2-point linear regression fit with linear axes. For the calculation of the regression curve, apply each signal of the standards (one obvious outlier of duplicates might be omitted and the more plausible single value might be used). The concentration of the samples can be read directly from the regression function. Sample signals above the highest standard have to be confirmed by a reference method. INTERPRETATION OF RESULTS: Various societies recommend different Cut-Off values for repetition of the measurement and the application of confirmatory assays. Depending on the application of samples from different populations, it is highly recommended that each laboratory establishes its own range of normal values and that this distribution of values is co-ordinated with the recommendations of the responsible society of this geographic region. 15. 1. KULLANIM AMACI: İdrar numunelerinde Serotonin, Gama-aminobutirik asit, Glutamik Asit, Histamin, 5-OH-L-Triptofan, Epinefrin, Norepinefrin ve Dopamin in LC-MS/MS yöntemiyle kantitatif analizi. 2. ÖZET VE AÇIKLAMA Adrenalin (epinefrin), noradrenalin (norepinefrin) ve dopamin ölçümü kronik özellikle hipertansiyon hastalarında feokromositoma tümörü taramasında önemlidir. Ayrıca bu test feokromositoma tümörüne rastalanan ve tedavi edilen hastalarda tümörün tekrar oluşumunun kontrolünde de önemlidir. Zivak İdrarda Neurotransmitters Analiz kiti Serotonin, Gama-aminobutirik asit, Glutamik Asit, Histamin, 5-OH-L-Triptofan, Epinefrin, Norepinefrin ve Dopamin in idrar numunlerinde doğru bir şekilde analizi için geliştirilmiştir. 3. TEST PRENSİBİ Neurotransmitters metodunda asidik ekstraksiyon ve deproteinizasyon işlemleriyle idrar numunesinden ekstrakte edilir. Ekstraksiyon işleminden sonra analitler reaktifler ile türevlendirilir. Türevlenmiş katekolaminler LC-MS/MS sisteminde analiz edilir. 4. UYARILAR VE ÖNLEMLER Sadece in-vitro kullanım amaçlıdır. ÇALIŞMA PROSEDÜRÜ Test prosedürünün yanlış ve değiştirilerek uygulanması test sonucunu etkileyebilir. Pipetleme hacimleri, inkübasyon zamanları ve hazırlık aşamaları tam olarak belirtilen miktarlarda ve talimatlara uygun bir biçimde yapılmalıdır. Teste bir kere başlandığı zaman, herhangi bir kesintiye uğramadan tamamlanmalıdır. Bunu sağlamak için gerekli reaktiflerin, maddelerin ve cihazların zamanında hazır olduğundan emin olunuz. Çalışmaya başlamadan önce bütün 20
reaktiflerin ve numunelerin oda sıcaklığına (18-25 C) gelmesini bekleyiniz, sıvı reaktifleri ve örneklerini kullanmadan önce hafifçe çalkalayarak karıştırınız. Reaktifler, pipetler ve numune hazırlık tüplerinde kirlilik ve bulaşma olmamasına dikkat ediniz. Her reaktif, standart ve numune için ayrı ve yeni pipet ucu kullanınız. Vial kapaklarını kendi aralarında değiştirmeyiniz. Kullanılmadığı zaman vial kapaklarını her zaman kapalı tutunuz. Vialler, vial kapakları, numune hazırlık tüpleri ve pipet uçları tek kullanımlıktır. İnkübasyon süreleri sonuçları etkilemektedir. Bütün numune hazırlık işlemleri belirtilen sırada ve sürede yapılmalıdır. 9. YÖNTEMDEKİ KISITLAMALAR Örnek toplama ve saklama koşullarının test sonuçları üzerindeki etkisi büyüktür. İdrar numuneleri dondurulmuş veya uygun bir asit çözeltisi ile asitlendirilmiş bir biçimde saklanmalıdır. (6N HCl, %50 Asetik asit, Na2CO3 (kristal), Borik asit (kristal)). İdrar numunesi berrak değilse, numune hazırlığından önce santrifüj edilmesi gerekmektedir. 10. TEST ÖNCESİ HAZIRLIKLAR Zivak Kalibratörün Hazırlanması : İdrar standart numunesine 2 ml 0,1 M HCl eklenip çözülür. Hazırlanan kalibratör 120 μl lik hacimlere bölünerek -20 C de saklanmalıdır. NUMUNE HAZIRLIĞI Numune Hazırlığı (Otomatik): 100 µl hasta numunesi (veya kalibratör ve kontroller) Zivak Multitasker Numune Hazırlama Tüpüne (Vialine) Alınır. Reaktifler ve yıkama solüsyonunun sisteme bağlandığından emin olunuz. MULTITASKER Analiz tablasına vialleri yerleştiriniz ve başlat butonuna basınız. Hazırlanmış numune 2-8 C de 24 saat boyunca stabildir. Numune Hazırlığı (Manuel): 100 µl hasta numunesi (veya kalibratör ve kontroller) durhaim cam hazırlık tüpüne alınır. 100 µl Reaktif 1 eklenir ve 10 saniye boyunca vortekslenir. 200 µl Reaktif 2 eklenir ve 10 saniye boyunca vortekslenir. 200 µl Reaktif 3 eklenir ve 10 saniye boyunca vortekslenir.5 dakika süresince beklenip tekrar 10 sn vortekslenir. 200 µl Reaktif 4 eklenir ve 10 saniye boyunca vortekslenir. 22
12000 x rpm de 3 dakika boyunca santrifüj edilir. 300 µl üst faz bir viale alınıp 3 L enjeksiyon yapılır Hazırlanmış numune 2-4 C de 24 saat boyunca stabildir. 12. KALİTE KONTROL Test sonuçları ancak verilen test talimatlarına uyulduğu takdirde geçerlidir. Kullanıcılar çalışırken GLP veya diğer kurumlar tarafından belirlenmiş olan kurallara ve standartlara bağlı kalmalıdır. Bütün standart ve kontrol değerleri kalite kontrol sertifikasında belirtilen aralıklarda olmalıdır. Değerler bu aralıkların dışında kaldığı takdirde analiz geçerli değildir ve tekrarlanmalıdır. Her laboratuvar daha ileri aşama kontroller için değeri bilinen numuneleri kullanmalıdır. Bu değerlerden sapma olması halinde reaktiflerin son kullanma tarihi, saklama koşulları, kullanılan pipetlerin doğruluğu, cihazlar, inkübasyon koşulları ve yıkama metotları gibi teknik detayların doğruluğundan emin olunmalıdır. Ayrıca uygun dış kalite kontrol programına üye olunması önerilir. 13. SONUÇLARIN HESAPLANMASI Standardın alanına karşı standart konsantrasyonu grafiği çizilir. Standart eğrisi lineer regresyon veya ağırlıklı lineer regresyon fonksiyonu ile hesaplanır. Regresyon eğrisini hesaplamak için, her bir standarda karşılık gelen sinyal değeri grafiğe alınır. Bilinmeyen konsantrasyonu regresyon eğrisinin fonksiyonundan direk olarak okunabilir. Standart eğrisinin üzerinde kalan konsantrasyon değerleri referans bir metot ile teyit edilmelidir. 23
14. SONUÇLARIN YORUMLANMASI Farklı toplumlarda farklı Cut-Off değerleri belirlenmelidir. Farklı toplumlardaki hasta numuneleri için yapılan çalışmalara bağlı olarak, her laboratuvarın kendi normal değerlerini belirlemesi önemle tavsiye edilir ve bu değerlerin dağılımı o coğrafi bölgedeki sorumlu kurumun önerdiği değerler ile uyumlu olmalıdır. Tanı ve tedavi aşamalarında bu analizlerin sonuçları klinik gözlemler ve tanı testleri ile desteklenmelidir. 15.
